Requesting a Copy of Consent

You will automatically receive a copy of the consent (CFS 431-B) via fax and email when the Clinical Services in Psychopharmacology consultant team and DCFS Guardian Consent Unit complete the processing of your request (CFS 431-A form). Our office, the CSP, provides consultation; only the DCFS Guardian or their authorized agents can issue and distributes consents.
