DCFS Office of the Guardian - Consent Unit
The DCFS Guardian serves as the acting custodian or legal guardian for certain youth in DCFS or juvenile court care. The Guardian Administrator makes welfare and legal decisions for youth in DCFS care.
The Guardian consents for the following:
- Routine medical and dental care
- Medical/surgical treatment
- Psychotropic medication
- Mental health treatment
- Travel (out of state, country, or extended trips)
- Release information
Approval and denial for medical requests (including psychiatric decisions) are informed by close collaboration with consulting medical experts.
**Youth over the age of 18 can consent for themselves, unless there is a court order stating that the DCFS Guardianship Administrator may continue to consent on their behalf until their 21st birthday.

When on the home page for the IDCFS Guardian consent portal, click the link entitled ‘Submit a Request for Consent.’
On the Requests for Consent page, there are seven links for different IDCFS forms. Above the links are general instructions, the hours when consents are processed, and some helpful numbers for assistance on completing the request forms.
To submit for psychotropic medications, click the link for CFS 431-A: psychotropic medication.

Use these resources especially if completing a one-time Emergency Medication notification because those need to be completed exactly as instructed, and follow different data-entry requirements.
Illinois Youthcare
YouthCare is a health insurance provider within Illinois. Some youth that are not in care of the state of Illinois have YouthCare memberships, especially when transitioning legal guardian rights to another individual or party. IDCFS Psychotropic Medication Consent is mandatory to complete YouthCare’s prior-authorization process for youth in care of the state of Illinois.
